This summary applies to persons hired to any of the following R.W. Scobie family of companies. R.W. SCOBIE, INC., Midwest General Agency, Premium Finance Corporation, Midwest General Brokerage, IMC Balloon, Employers Benefit Group, First Western Insurance and Tower Special Facilities, INC.

Health Insurance • UnitedHealthcare Definity HDHP/HSA Plan
• Employer and employee cost share
• Eligible if regular, full-time status
• Eligible 1st of the month after 30 days of employment
Life Insurance • Term life insurance
• Employer pays the full cost
• Employee scheduled to work 27 hours per week
• Coverage 1 times annual salary (minimum $25,000; maximum $100,000)
• Spouse coverage $2,500; dependent coverage $2,000
• MEDEX Travel Assistance Program
• Additional coverage is available (paid by employee)
Long Term Disability Insurance • Employer pays the full cost
• Replaces 60% of gross income to $6,000 per month for a qualified
  disability
• Includes Horizon Health Employee Assistance Program
• Employee scheduled to work 27 hours
• Eligibility 1st of the month after 30 days of employment
401(k) Plan • Managed by M&I Trust
• Company matches 100% of the first 3% and 50% of the next 2%
• You may contribute up to the IRS maximum
• Eligibility after 3 full calendar months of employment on the next
  enrollment date (3/1, 6/1, 9/1, 12/1) - must be age 20 and a regular
  employee
Dental Insurance
(voluntary)
• Employee pays full cost
• Employee scheduled to work 30 hours
• Eligibility 1st of the month after 30 days
Flex Plans • Medical Reimbursement Plan - deductions may be withheld from your
  check on a pre-tax basis up to $5,000 (if not enrolled in the company
  health insurance plan)
• Dependent Care Reimbursement Plan - deductions may be withheld from
  your check on a pre-tax basis up to $5,000
• Employee scheduled to work 1000 hours/year
• Eligibility 1st of the month after 30 days
Vacation • Eligibility if regular full time status
• Calculated based on length of employment as determined by anniversary
  date of hire
• Vacation accrual key time frames:
Between 0-5 years = 10 days
Between 5-10 years = 15 days
Between 10-15 years = 20 days
After 25th year = 25 days
• May be taken in increments as little as ½ day or as much as 2 full weeks
Personal Time Off
(non-exempt employees)
• Used for unscheduled absences such as injury, illness or
   medical appointments
• Eligibility after 6 full calendar months of employment
• Accrue ½ day per month
• May be taken in 2 hour increments
• Program pro-rated for part-time employees
Holidays • Generally closed on:
New Year's Day
Good Friday (at noon)
Memorial Day
Independence Day
Labor Day
Thanksgiving Day
Friday after Thanksgiving
Christmas Day
• One Personal Choice Day, if not assigned to be used in conjunction with
  a holiday
• One Birthday Holiday
• Eligibility if regular full-time status
• Program is pro-rated for part-time employees
Health Club Reimbursement • Covers health club initiation fee up to $15
• Covers health club monthly fee up to 50% of the monthly fee
   - maximum of $20 per month ($10 per month for part-time employees)
Smart Buck Program • Recognition and reward program
• Manager presents Smart Buck to employee
• Employee may redeem Smart Buck toward gift certificates,
   company gift or vacation
Educational Bonus • Employee obtains bonus after completion of the course and passing
   the test
• Bonus varies dependent upon the course or designation
